Family law rules amendments.

The Florida Supreme Court recently adopted an amendment to Florida Family Law Rule 12.015 and adopted new Florida Family Law Rule of Procedure Form 12.996(d), as proposed by the Family Law Rules Committee. The new form is a Florida addendum to a federally approved income withholding form, "Income Withholding for Support," OMB 0970-0154. The amendment to rule 12.015 adds new form 12.996(d) to the list of Florida Family Law Rules of Procedure Forms. The court invites all interested persons to comment on these amendments, which are reproduced in full below, as well as online at www.floridasupremecourt.org/decisions/opinions.shtml. An original and nine paper copies of all comments must be filed with the court on or before August 27, with a certificate of service verifying that a copy has been served on Matthew Burns Capstraw, Chair, 165 W. Jessup Avenue, Longwood 32750, and on the Bar staff liaison to the committee, Ellen Sloyer, 651 East Jefferson Street, Tallahassee 32399-2300, as well as a separate request for oral argument if the person filing the comment wishes to participate in oral argument, which may be scheduled in this case. The committee chair has until September 18 to file a response to any comments filed with the court. Electronic copies of all comments also must be filed in accordance with the Court's administrative order In re Mandatory Submission of Electronic Copies of Documents, Fla. Admin. Order No. AOSC04-84 (Sept. 13, 2004).

RULE 12.015. FAMILY LAW FORMS

(a) Forms Adopted as Rules. The forms listed in this rule shall be adopted by the rulemaking process in Fla. R. Jud. Admin. 2.140. The Family Law Rules Committee of The Florida Bar shall propose amendments to these forms and any associated instructions. These forms shall be designated "Florida Family Law Rules of Procedure Forms." Forms coming under this provision are:
